In a way, you could think of All Saints' Hospital as being a lot like its emergency room nurse Jackie Peyton.
From the outside, both seem pretty put-together, efficient, and professional, but pay any attention and you'll see the cracks in the founation. While Jackie has been nursing a serious prescription drug addiction for the past three seasons, All Saints' has ran into a lot of bureaucratic red tape, losing their chapel statues and a chance to meet the First Lady last season. Considering the way things have been going, it's about time for a change, no?
In the fourth season premiere of Nurse Jackie, title "Kettle-Kettle-Black-Black", Jackie has officially hit her rock bottom and is going for professional help. Yes, you read that right. No botched intervention, no last minute Get Out of Rehab Free card from Akalitus, no reset button - it's about to get very real very quickly for Jackie. Last season the show hinted that she wanted to change but had gotten too deep into her addiction to do it alone, which is doubly hard since Nurse Jackie Peyton is the last person to ask for help. She's made it farther than I ever thought she would, simply by agreeing to be in rehab, but the real work is beginning, particularly for every employee at her facility.
Elsewhere on Nurse Jackie, All Saints' has been taken over by a corporation (including guest star/new boss Bobby Cannavale), O'Hara spills a secret, and Jackie's biggest lie yet gets uncovered by the person she's been fooling for so long.
